New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Please bring along two forms of identification to the surgery, i.e. passport, driving licence and a utility bill.

If you complete an Online Registration Form we will still need you to call at the surgery to complete the opening formalities, bringing with you two forms of identification as detailed above.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.  

All new patients are asked to complete a basic Health Questionnaire which is then assessed by one of our staff. Medical treatment is available from the date of registration. Please contact reception for further information.
